I am a General Practitioner - an Australian. Qualified 1914. Have served as Lieut in RAMC for 12 months -- period of service ending May 12, 1916. Have been confined to bed for nine weeks and am now convalescing. At present suffering from Epidemic Parotitis [mumps] and was refused acceptance for a second period of 12 months on account of my illness being protracted.
