Scottish Medical Service Emergency Committee, I am prepared to render the service
or services marked above. This offer is subject to the condition that, in the event
of such service requiring me to leave my present work, I am enabled to make
arrangements for having it carried in during my absence.

Additional Information: 

Note next to part time military work: I am absolutely willing to do some part time work but my present duties leave little spare time. I feel also that there are younger men in the district with fewer ties. I am married and have two children. Nevertheless if the necessity arises I will do anything I can.
